Consent Statement

I, the undersigned, in exchange for good and valuable consideration, hereby consent and grant to Caroline Maguire d/b/a New England Coaching Services (“NE Coaching”) and its designees the following rights:

1. To take, modify, and utilize classroom photographs, audio, video and/or digital recordings that may contain my voice, image, and/or likeness (the “Materials”); and
2. To display, modify, edit, and otherwise utilize the Materials for educational or teaching purposes including:  (a) for download or streaming from NE Coachingor third party web sites for educational and teaching purposes; (b) for use by NE Coaching employees and current and potential clients; and (c) at NE Coaching sponsored or co-sponsored events;

In granting this consent, I understand and agree that:

1. a) the Materials may or may not contain attributions specifically identifying me;
2. b) statements I may make in the Materials reflect my honest opinions or experience;
3. c) I have no right of approval and no right to inspect or preview the Materials and any derivative works thereof, or any use to which they may be put;
4. d) I have no claim, now or in the future, to any compensation, attribution or other similar right relating to or arising out of the Materials or their use as described in this Agreement.

On behalf of myself, my heirs and any of my representatives and assigns, I hereby waive, discharge, and release NE Coaching and its officers, directors, employees, agents, representatives, successors and assigns, and those acting under their authority from any and all rights, demands, losses, liabilities, claims and causes of action whatsoever which I may now or hereafter be entitled to assert relating directly or indirectly to the Materials.  I further hereby release and discharge NE Coaching and its employees, agents, successors, and assigns from any and all claims I might have in connection with the use, display, dissemination or exploitation of the Materials, including, but not limited to, any claims for defamation, invasion of privacy, right of publicity, rights in my name, image, voice or likeness, and/or violation of any moral or artist rights.

I represent that I have read and understood the contents of this release and consent.  I acknowledge that, if I am under eighteen (18) years of age, the consent and signature of my parent or legal guardian is also required for this release.
